Let me tell you this first —Not all bleeding in pregnancy means something is wrong. But every episode of bleeding deserves medical evaluation.

How Common Is Bleeding in Pregnancy?

Many women are surprised when I tell them:

  • Nearly 1 in 4 pregnancies can have some bleeding in the first trimester.

  • Light spotting does not always mean pregnancy loss.

  • However, heavy bleeding or pain should never be ignored.

During the first 12 weeks, bleeding is more common than you think.

1. Implantation Bleeding

Sometimes, when the embryo attaches to the uterus lining, light spotting can occur.

  • Pink or brown discharge

  • Very light

  • Lasts 1–2 days

  • No severe pain

Many patients mistake this for a light period.

2. Subchorionic Bleed

This is a small blood collection near the pregnancy sac.

  • May show up on ultrasound

  • Can cause mild to moderate bleeding

  • Often resolves with monitoring and rest

With timely follow-up, many women go on to have completely healthy pregnancies.

3. Threatened Pregnancy Loss

Heavier bleeding with cramps may indicate risk.

  • Bright red bleeding

  • Clots

  • Lower abdominal pain

Some pregnancies continue with proper medical supervision. That is why early evaluation is critical.

4. Ectopic Pregnancy (Medical Emergency)

An ectopic pregnancy occurs when the pregnancy develops outside the uterus.

Warning signs I always tell my patients to watch for:

  • Severe one-sided lower abdominal pain

  • Shoulder pain

  • Dizziness or fainting

  • Heavy bleeding

This condition can be life-threatening if not treated urgently. If you experience these symptoms, go to the nearest emergency facility immediately.

Bleeding in Later Pregnancy (Second & Third Trimester)

Bleeding after 20 weeks is less common but often more serious.

Placenta Previa

  • Painless, bright red bleeding

  • Placenta partially or completely covering the cervix

  • Requires strict medical monitoring

Placental Abruption

  • Severe abdominal pain

  • Back pain

  • Dark bleeding

  • Tight, tender abdomen

This is an emergency and requires immediate hospital care.

When Should You See a Gynecologist Immediately?
